RAMP Application

Thank you for your interest in the Cal Poly Pomona Reading, Advising, & Mentoring Program (RAMP/TRIO Student Support Services). The information requested in this application is necessary to maintain compliance with the U.S. Department of Education's record keeping and reporting requirements. The information you supply will be used to determine your eligibility for the program and will remain confidential.
Please complete this application as thoroughly as possible. You will not be able to save and return to your application if you close it. You must click "Submit Application" for your application to be received. You may want to review the application questions, make notes, and gather the required documents before beginning to complete the application. If you have any questions, please email the RAMP staff at ramp@cpp.edu.
To be eligible, you must meet the following requirements:
- citizen or national of the United States or meet the residency requirements for Federal student financial assistance
- enrolled at CPP or accepted for enrollment for the next academic term
- need for academic support (we will work with you to determine your need)

First-Generation College Student Status
The Higher Education Act of 1965 and 1998 defines the term "first generation college student" as "an individual both of whose parents did not complete a bachelor's degree; or in the case of students who live with and are supported by only one parent, a student whose only such parent did not complete a bachelor's degree."
To establish if you are a first-generation college student, please answer the following questions regarding your parents' educational level. If you live with and are supported by only one parent, answer "Other" for the other parent's educational level. If your parent received a degree from another country and it is not recognized in the U.S., answer "Other" for that parent. If you are unsure how to answer the question, email ramp@cpp.edu for guidance.

Program CommitmentÂ
Once you are accepted into RAMP, you are a participant until you graduate from Cal Poly Pomona. To foster your success and receive services provided by RAMP/TRIO SSS, we ask that you commit to the following: Â
- I will have contact with RAMP at least two times per semester until I graduate from CPP. One of these contacts will include a meeting with my RAMP advisor each semester. Â
- I will use campus tutoring services and resources as needed or per the recommendation of an instructor or RAMP advisor. Â
- I will complete the Free Application for Federal Student Aid (FAFSA) each year as needed.Â
- I will notify the RAMP staff of changes in my name or contact information. Â
- I will read my CPP email on a regular basis.Â
- I will meet with my RAMP advisor if I am considering withdrawing from school. Â
- If I am no longer interested in being a RAMP participant, I will contact the office and complete an exit interview with my program advisor. Â

Media Release
While participating in RAMP activities and events, photos, video, and audio recordings of participants may be taken and/or related textual information may be made and collected that contain you, your name, identity, voice, and/or likeness. These media are typically used to document memories and promote RAMP. RAMP may include such media regarding your participation in the program in publications, including digital materials and emails. Please indicate below your consent and agreement for RAMP to use and reproduce any photographs, video, audio, and textual information taken of you or provided by you for program materials, website, print, social networking sites, and other media for promotional or publication purposes without compensation. Â

Authorization and Affirmation
I hereby authorize RAMP staff to obtain academic, financial aid, and other information pertinent to my participation in RAMP. I understand that my application will be kept on file and that resulting information received from advisors, admissions, financial aid counselors, instructors, or other campus staff will be kept confidential in compliance with the Family Educational Rights and Privacy Act.
